29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):     a)  Unit 56,  on or about January 21, 2016, three employees were exposed to fall hazard while performing work from the 2nd, 3rd and 4th levels of a tubular welded frame scaffold.  These sections of the scaffold were not fully planked, thus exposing the employees up to a 24-foot fall hazard.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph:    a)  Unit 56,  on or about January 21, 2016, three employees used a 4 tier scaffold, 24 feet in height and 5 feet in width, that was not restrained from tipping over, exposing employees up to a 24-foot fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:     a)  Unit 56,  on or about January 21, 2016, three employees were observed working from a tubular welded frame scaffold without a ladder or any other means of safe access/egress to reach the upper levels of the scaffold, exposing employees up to a 24-foot fall hazard.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities were not performed only by experienced and trained employees selected for such work by the competent person:     a) Unit 56, on or about January 21, 2016, the tubular welded frame scaffolding that employees work from was not equipped with guardrails, access, full floor planking, and not restrained from tipping, and an qualified competent person was not supervising and directing the erection, moving, and altering of the scaffold.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:     a)  Unit 56,  on or about January 21, 2016, three employees were working from a tubular welded frame scaffold without guardrails and no other means of fall protection, thus exposing them up to  24-foot fall hazard.
