29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:  a) East side of the building, on or about March 29, 2016, two employees were elevated and working from an unapproved work platform that was supported by a Skyjack JLG high reach forklift 8042, #615349.  The forklift manufacturer does not allow this forklift to be used to elevate personnel or be fitted with any form of personnel platform, which exposed employees to fall hazards up to 9 feet.

29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area.
