(a) On or about 1/21/2016, at work site at 184-60 Jamaica Avenue, Jamaica, NY 11423 Housekeeping practices were inadequate to prevent accumulation of combustible wood dust in the woodworking room. At least 7% of the room, including floor, shelves, window sills, window caulking, light fixtures and other horizontal surfaces were covered in wood dust at a depth of greater than 1/32 inch. ABATEMENT DOCUMENTATION REQUIRED PURSUANT TO 29 CFR 1903.19.
