29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) As occurring on 01/14/2016, an employee of Taylor and Fulton Packing House sustained an injury to his foot while unloading material from a trailer with a forklift. The employee was admitted as an in-patient, hospitalized overnight, and treated for his injury. The company learned about the hospitalization on 01/14/2016 and failed to report the hospitalization to OSHA until 01/18/2016.
