29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about January 20, 2016, and at times prior thereto, the employer did not ensure point of operation guarding was in place on the following press brakes: a) Chicago Press Brake #1 b) Chicago Press Brake #2 c) Chicago Press Brake #3. Employees were exposed to the hazard of being caught in the press brake point of operation. Refractory Anchors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910. 212(a)(3)(ii), which was contained in OSHA inspection number 980792, citation number 1, item number 1b and was affirmed as a final order on August 13, 2014, with respect to a workplace located at 9836 S 219th East Ave., Broken Arrow, OK 74014.
