29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Southside of building: On or about January 20, 2016 employees were exposed to struck-by hazards, while hoisting wall panels that were rigged to a JLG Forklift, Model 8042, which had a Viavac hoisting device attached to the forks of the forklift. The lifting capacity of the forklift was not de-rated with the addition of the hoisting device.

Southside of building: On or about January 20, 2016 employees were exposed to struck-by hazards, while hoisting wall panels that were rigged to a JLG forklift, Model 8042, which had a Viavac hoisting device attached to the forks of the forklift. The attachment was not approved by the forklift manufacturer.
