29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 01/22/16, at the Autumn Woods Housing Project, located on 155 Sunset Center Drive, in Brockport,, New York, employees were working on surfaces 20 feet above ground level; with unprotected sides, and were not using fall protection. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about , New York, a portable extension ladder in use was not positioned so that the side rails extended at least 3 feet above the access level. The ladder was not tied off at the top and the access point was 20 feet above the lower surface. NO ABATEMENT CERTIFICATION REQUIRED
