29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 11/06/19, at 54 University Avenue, in Rochester, New York, employees were working on surfaces 22 feet above lower levels without the use of fall protection. The employer did not enforce the use of fall protection ABATEMENT DOCUMENTATION REQUIRED The Above the Rest Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(11), which was contained in OSHA inspection number 1119807, Citation number 1, Item number 1 and was affirmed as a final order on 05/02/16, with respect to a workplace located at 155 Sunset Center Drive, Brockport, NY

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 11/06/19, at 54 University Avenue, in Rochester, New York, a portable extension ladder in use was not positioned so that the side rails extended at least 3 feet above the access level. The ladder was not tied off at the top and the access point was 22 feet above the lower surface. NO ABATEMENT CERTIFICATION REQUIRED The Above the Rest Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053 (b)(1), which was contained in OSHA inspection number 1119807, Citation number 1, Item number 2 and was affirmed as a final order on 05/02/16, with respect to a workplace located at 155 Sunset Center Drive, Brockport, NY.
