29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training , and periodic inspections, to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4). On or about 19 January 2016 the employer did not ensure that employees engaged in changing a blade on the Brewer Inc (Model GE 2000 HD 2 Serial 111700) resaw were protected by a Energy Control Program. Employees were exposed to amputation and/or struck-by hazards.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to this standard when the employer determined that any voluntary respirator use was permissible. The employer did not ensure that employees engaged in sawing lumber with the Brewer Inc (Model GE 2000 HD 2 Serial 111700) resaw and voluntarily using N95 type respirators while exposed to respiratory hazards were provided the information contained in Appendix D of 29 CFR 1910.134.
