(a)    On or about 01/22/16, at the Autumn Woods Housing Project, located on 155 Sunset Center Drive, in Brockport, New York,  employees installing insulation on the second floor of a townhouse were not protected against falls through unprotected window openings.  Employees were exposed to falling 15 feet to ground level.       ABATEMENT DOCUMENTATION REQUIRED
