29 CFR 1910.178(l)(2)(i)(A): Trainee(s) were operating powered industrial trucks without the direct supervision of persons who had the knowledge, training, and experience to train operators and evaluate their competence. a) Warehouse Area (Aisle 101, Rack #126/124) - On or about January 18-19, 2016, a new employee was in the process of being trained to operate a Hyster electric double pallet jack end rider (Model B80ZHD, SN A257N01735K) under the direct supervision of the employer's designated powered industrial truck trainer. The trainee, who had no prior training or experience in operating the double pallet jack, was instructed to continue practice operations in order to improve proficiency in making turns with the vehicle. The trainee continued operating the vehicle without the direct supervision of a competent person, exposing the trainee to amputation and crush hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.178(l)(2)(i)(B): Trainee(s) were operating powered industrial trucks where such operation endangered the trainee or other employees. a) Warehouse Area (Aisle 101, Rack #126/124) - On or about January 18-19, 2016, a new employee was in the process of being trained to operate a Hyster electric double pallet jack end rider (Model B80ZHD, SN A257N01735K). The trainee was instructed to continue practice operations in order to improve proficiency in making turns with the vehicle. The trainee was operating the vehicle in an aisle with industrial storage racks on both sides of the aisle, which restricted space for maneuvering and exposed the trained to amputation and crush hazards in the event the vehicle came into contact with the racks. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
