29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: (a) Located on the floating barge at 101B Youngs Bay Bridge in Astoria, Oregon: On or about January 19, 2016 the inspection revealed that employees were potentially exposed to serious injury due to the use of a damaged flexible cord that was cut to the conductor. The cord was being used to power the dive monitor.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) Located on the floating barge at Young's Bay Bridge in Astoria, Oregon: On January 19, 2016 the inspection revealed a flexible cord with the outer insulation pulled back from the plug, exposing the inner insulated wires.
