When diving from surfaces other than vessels in areas capable of supporting marine traffic, a rigid replica of the international code flag "A" at least one meter in height was not displayed at the dive location in a manner which allows all-round visibility.  (a) Burnside Bridge Pier #1 - Cascade Dive Co Barge - On or about August 14th, 2019 and at times prior thereto, employees were exposed to river wake from passing boat traffic without the international code flag "A" flying on the barge.
