29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified:  (a)    Facility, One Pork Avenue, Tipton, PA:  On or about January 13, 2015 the employer had not provided appropriate hand protection to employees who clear jams from the grinder.  An employee suffered an amputation of the tip of their ring finger when it was struck by a blade when the employee was manually moving the rotary knives device.  The gloves being used by the employee was not of a construction what would have provided adequate protection to prevent the amputation.

29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s):  (a) Facility, One Pork Avenue, Tipton, PA:  On or about January 21, 2016 lockout devices used for the grinder and fill machines did not indicate the identity of the employee(s) applying the device.  The same lock is used for both machines and there were no tags or other identifying means to indicate which employee had applied the device.
