29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Facility, One Pork Avenue, Tipton, PA: On or about September 6, 2108 no guards were in place to prevent employees from entering into the area of the elevator that is triggered by an optical eye. Employees may enter the area during the clearing of a jam and can activate the elevator if movement is picked up by the eye. An employee entered into the area of the elevator and activated the eye causing the elevator to rise and pin the employee against an upper platform.
