29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:    750 W. Berkley Ave., Norfolk, VA 23523: On or about January 28, 2016, and times prior thereto, standard railings did not have a vertical height of 42 inches above a platform:    a) Electrical Shop: The stairway to the second floor had a platform at a height approximately 13 feet above the first floor, with a top rail vertical height of approximately 32 inches above the platform, exposing workers to fall hazards.    No abatement certification or documentation required.

Berkley Ave., Norfolk, VA 23523: On or about January 28, 2016, and times prior thereto, employees were not protected from falls from fixed ladder landing platforms by standard railings, exposing workers to fall hazards.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abathement.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    750 W. Berkley Ave., Norfolk, VA 23523: On or about January 28, 2016, and times prior thereto, exit routes were not kept free and unobstructed:    a) Boiler Room: access to the rear exit door was obstructed by materials such as, but not limited to, a frame scaffold.    b) Electrical Shop: access to the two exit doors was obstructed by the support posts of a hoist such that the front exit door access was limited to a 22-1/2" opening, and the rear exit door access was limited to a 24" opening.    c) Pipe Shop/Electrical Shop: the exit route on the second floor between the electrical shop and the pipe shop was obstructed by placement, at approximately waist height, of a 2x4 board across the double doors in the electrical shop. A 2x4 board was available next to the double doors in the pipe shop and supports for the board were in place for the board at approximately waist height, such that the double doors could be obstructed from both shops, exposing workers to potential injuries in the event of an emergency evacuation.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
