29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100°F (37.8 °C), were dispensed into containers without the nozzle and container being electrically interconnected. a) BAE Systems Norfolk Ship Repair fuel farm: On or about 01/28/2016, employees transfer gasoline with a flash point of -45° F from a 1000 gallons tank equipped with a nozzle to a metal gas without being electrically interconnected, as a result employees were exposed to possible in burns. No abatement certification or documentation required.

29 CFR 1910.213(h)(4): Radial saw(s) were not installed in a manner so as to cause the cutting head to return gently to the starting position when released by the operator: a) Carpenter Shop: On or about 02/01/2016, the Delta Industrial Radial Arm Saw serial #HPJ56B3409HP used in cutting wood the cutting head did not return to the starting position, therefore employees were exposed to a possible amputation hazard. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) 750 W. Berkley Ave., Norfolk, VA 23523, Structural Fabrication Shop: On or about 02/01/2016, the Jet horizontal band saw model #HBS-916W, serial #14094204 used for straight cuts, 4 inches of unused saw blade was not guarded, as a result employees were potentially exposed to a laceration hazard. b) 750 W. Berkley Ave., Norfolk, VA 23523, Carpentry Shop: On or about January 28, 2016, and times prior thereto, the manual control wheel which rotated when the L. Power & Company compound band saw was in operation was not guarded, exposing workers to caught-in injuries. BAE Systems Southeast Shipyard was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 982935, citation number 1, item number 2 and was affirmed as a final order on 09/10/2014, with respect to a workplace located at Main Gate Dunlap Drive, Mobile Alabama. No abatement certification or documentation required.
