Mercer Forge Corporation, 200 Brown Street, Mercer, Pa, 16137, Quonset Hut area adjacent to Lunch room area, On or about January 26th, 2016, and times prior thereto: An electrical panel box was not effectively closed, in that, portions of the panel box were open potentially exposing the internal components. Two openings on the left side of the box were not effectively closed.
