29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures:  a)  Mailcrafters Model #9800 Inserter, id #EI0222:  Lockout procedure for the Mailcrafters 9800 #EI0222 serial # 2702212730 does not include any specific requirements for verifying total de-energization of all energy sources, on or about 01/27/2016.   NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii):  a)  Mailcrafters Model #9800 Inserter, id #EI0222:  While Mailcrafters Model #9800 Inserter, sr #2702212730 was running, employee reached in on the lower left hand side of and between the between the upper sprocket (approximately 2 1/8" diameter) and the drive belt to retrieve a socket, on or about 01/27/2016.   NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
