29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) Employees who performed servicing and maintenance on and around the Washer Line 1-2 were not required to: (1) Physically locate and operate the electrical disconnect in such a manner as to isolate the machine or equipment from the energy source. (2) Affix a lockout or tagout devices to the electrical disconnect.

29 CFR 1910.147(f)(1): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed: Employees performing testing or positioning of machines, equipment did not perform the following: (1) remove the equipment from the machine or equipment area in accordance with(e)(2) (2) deenergize all systems and reapply energy control measures in accordance with paragraph (d) to continue with servicing and/or maintenance. (a) On January 21, 2016 while checking/trouble shooting the Washer Line 1-2 for air leaks, the machine was not isolated to prevent from start ups.

29 CFR 1910.212(a)(5): Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch. a) On or about January 21, 2016 while checking/trouble shooting the Washer Line 1-2 for air leaks, guarding was not in place to prevent employees exposure to rotating fan blades.
