One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.      a) The Shakuri Rotary Mill 1500 Line 2-17 allows employees to enter into the cutting area due to the guard having an opening that exposed operators hand to reach the rotating cutting head due to the door not having a adequate guarding method such as but not limited to either barrier guards  to prevent entry or electronic safety devices that would stop the motion of the mill when the door was open.

The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    a) On August 22, 2018, employees who removes jams from the Shakuri Rotory Mill 1500 line 2-17 were not required to:  (1) Turn off and shut down the Mill Head. (2) Physically locate and operate energy isolating devices in such a manner as to isolate machine or equipment from the electrical energy sources. 3) Affix a lockout or tagout devices to energy isolating device(s).  The Ahresty Wilmington Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR1 1910.147(d), which was contained in OSHA inspection number #1120609, citation number (1), item number (1) and was affirmed as a final order on April 12, 2016, with respect to a workplace located at 2627 S. South St, Wilmington, OH 45177.
