29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a)  On or about January 27, 2016 and at times prior to, employees were exposed to caught-in hazard where the employer failed to ensure that lockout and tagout procedures were utilized to protect employees performing servicing and maintenance on machinery, such as but not limited to cleaning dust from a rotary valve underneath a dust collector on EVA Mezzanine.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees.  a)  On or about January 27, 2016, and at times prior to, the employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees who clean out dust from a rotary valve underneath a dust collector on EVA Mezzanine.  Employer did not have procedure for cleaning dust collector.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a)  On or about January 27, 2016, and at times prior to the employer did not provide effective information and training for employees use chemical products such as but not limited to IRGANOX 1076 FD, Triallyl Isocyanurate, Curox Solar SC, Xiameter(R) OFS-6030 Silane etc., and polymers and put them into mixing tank to make solar panel protection sheets, resulting in a chemical hazard.

The entry does not describe the injury in detail in column (F).  c)  On or about 1/21/2016, Mix Assistant had the middle and ring fingers of the left hand severed.
