29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: (a) PRODUCTION AREA, BAND SAW ROOM - Employees used the band saw to dismantle pallets, and the exit route from the area where employees stand and perform work behind the band saw was reduced to approximately 14 inches by a tote of scrap wood that had been placed next to the saw, thereby exposing employee(s) to possible injury, on or about January 22, 2016. No abatement certification or documentation required.

29 CFR 1910.178(q)(7): Industrial trucks were being placed in services after the examination showed condition(s) adversely affecting the safety of the vehicle: (a) PRODUCTION AREA - Employees operated the Toyota 5FGC15 forklift, and the forklift was observed in operation while the right tie-down strap for the propane fuel cylinder was broken, thereby exposing employees to possible injury, on or about January 22, 2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
