Wyandot, Inc. - Marion, OH: On or about January 28, 2016, employees were exposed to injuries associated with rotating parts in that horizontal shafting associated with the dough roller motor located in the fab room was not guarded.    b. Wyandot, Inc. - Marion, OH: On or about January 28, 2016, employees were exposed to injuries associated with rotating parts in that horizontal shafting associated with the inclined conveyor, adjacent to the dough roller in the fab room, was not guarded.    c. Wyandot, Inc. - Marion, OH: On or about January 28, 2016, employees who clean the floor area of Tort Line 1 were exposed to injuries associated with rotating parts in that horizontal shafting associated with the line's seasoner tumbler was not adequately guarded.    d. Wyandot, Inc. - Marion, OH: On or about January 28, 2016, employees who clean the floor area of Tort Line 2 were exposed to injuries associated with rotating parts in that horizontal shafting associated with the line's seasoner tumbler was not adequately guarded.

Wyandot, Inc. - Marion, OH: On or about January 28, 2016, a projecting shaft end located on the inclined conveyor of the fab room sheeter exposed employees to caught-by hazards in that the rotating part was not guarded.    b. Wyandot, Inc. - Marion, OH: On or about January 28, 2016, a projecting shaft end located at the end of the product conveyor, following the Tort Line 1 seasoning tumbler, exposed employees to caught-by hazards in that the rotating part was not guarded.    c. Wyandot, Inc. - Marion, OH: On or about January 28, 2016, a projecting shaft end located at the end of the product conveyor, following the Tort Line 2 seasoning tumbler, exposed employees to caught-by hazards in that the rotating part was not guarded.

Wyandot, Inc. - On or about January 28, 2016, the employer did not ensure that the chains and sprockets, used to turn the seasoning tumbler at Tort Line 1, were adequately guarded to protect employees from being injured by the moving parts.   b. Wyandot, Inc. - On or about January 28, 2016, the employer did not ensure that the chains and sprockets, used to turn the seasoning tumbler at Tort Line 2, were adequately guarded to protect employees from being injured by the moving parts.

Wyandot, Inc. - Marion, OH: On or about January 28, 2016, employees were exposed to electrical hazards in that parts which protect electrical equipment of the plug attachment, including the plastic housing and locking connector and water tight for the Tort Line 2 tumbler were damaged.      c. Wyandot, Inc. - Marion, OH: On or about January 28, 2016, employees were exposed to electrical hazards in that the locking connector and water tight seal plug attachment for the Tort Line 2 oil over-sprayer was removed.

Wyandot, Inc. - Marion, OH: On or about January 28, 2016, the employer did not ensure that the locking type connector plug, used to power a tumbler at Tort Line 1, was properly secured.  The attachment plugs twist locking device was damaged which prevented a secure connection.   b. Wyandot, Inc. - Marion, OH: On or about January 28, 2016, the employer did not ensure that the locking type connector plug, located at the Seasoner VFD disconnect and used to power a tumbler at Tort Line 2, was properly secured.  The attachment plugs twist locking device was damaged which prevented a secure connection.   c. Wyandot, Inc. - Marion, OH: On or about January 28, 2016, the employer did not ensure that the locking type connector plug, used to power the Tort Line 2 oil over-sprayer, was properly secured.  The attachment plugs twist locking device was removed, which prevented a secure connection.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    a. Wyandot, Inc. - Marion, OH: On or about January 28, 2016, the employer did not assure that 480v flexible power cords for portable equipment such as, but not limited to, Tort Line tumblers and oil over-sprayers, were inspected for damage before being placed in service.  Damage to equipment cord and plugs expose employees to electrical hazards.
