29 CFR 1910.178(f)(2): The storage and handling of liquefied petroleum gas fuel was not in accordance with NFPA Storage and Handling of Liquefied Petroleum Gases (NFPA No. 58-1969), which is incorporated by reference as specified in 29 CFR 1910.6: Reference NFPA No. 58-1969 Division IV 4.3.(c) (a) Location - 80 Concord Street, North Reading, MA: On or about 1/28/2016, removable propane fuel cylinders powering forklifts were not securely mounted to prevent jarring loose, slipping, or rotating as required by NFPA No. 58-1969 Division IV 4.3.(c).

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: (a) Location - 80 Concord Street, North Reading, MA: On or about 1/28/2016, forklift P1584 had no means to secure its liquid petroleum gas fuel cylinder in place, and was not removed from service. UPS Freight was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(1), which was contained in OSHA inspection number 1015873, citation number 2, item number 1 and was affirmed as a final order on 4/30/2015, with respect to a workplace located at 1700 Tower Drive, Kaukauna, WI 54130. UPS Freight was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(1), which was contained in OSHA inspection number 980950, citation number 2, item number 1 and was affirmed as a final order on 12/11/2014, with respect to a workplace located at 1370 Goodyear Drive, El Paso, TX 79936.
