29 CFR 1926.502(d)(6)(i): A snap hook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself: a) On or about January 28, 2016 at 2101 Berry Hill Rd., Montgomery, Al., building 13: The employer did not ensure employees using personal fall arrest were properly anchored in that their rope was wrapped around a support beam and the snap hook was connected back to the rope while taking measurements and installing sheathing which exposed them to falls of 30 feet.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) On or about January 28, 2016 at 2101 Berry Hill Rd., Montgomery, building 13: The employer failed to ensure employees taking measurements and installing sheathing on the roof of a residential apartment building adjusted their rope grab as they moved allowing additional slack to accumulate allowing a fall of more than 6 feet.
