(5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: Worksite located at Renee 2-11 H2. Carter County, OK. On or about February 01, 2016, and at times prior there to, the employer was lifting personnel to service the derrick with an air winch that was not approved for personnel lifting, exposing employees to caught-in and fall hazards.
