29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: a) South east area of shop - on or about February 10, 2016, employees operated a Hyd-Mech horizontal band saw with model number S-20, but approximately 12 inches of the unused portion of the blade was not guarded. b) South area of shop - on or about February 10, 2016, employees operated a Milwaukee vertical band saw with model number 6232-20, but the saw was not equipped with guarding for the unused portions of the blade. c) South area of shop - on or about February 17, 2016, employees operated a three roll-forming machine manufactured by BCR with model number RBM 2-30, but no guarding was provided to protect the operators from the in-feed sections of the machine. d) South west area of shop - on or about February 17, 2016, employees operated horizontal lathe manufactured by J.J. McCabe Machinery with model number 2306, without chuck guards to protect the operator from injury when operated with the metal arms extending past the periphery of the chuck.
