29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a.) Small Outside Garnet Blasting Booth - On or about October 11, 2017, The Employee, who was abrasive blasting iron with garnet was exposed to continuous noise at 180.6% of the exposure level of 90 dBA or an eight hour time weighted average (TWA) noise exposure of 94.3 dBA. This exposure was observed occurring over a 423 minute sampling period on 10/11/17. A zero exposure was assumed for the 57 minutes not sampled. The employer did not obtain an annual audiogram for the employee exposed above the Action Level of 85 dBA.

29 CFR 1910.29(b)(10): The employer did not ensure when guardrail systems used at hoist areas, a removable guardrail section, consisting of a top rail and midrail, are placed across the access opening between guardrail sections when employees are not performing hoisting operations. The employer may use chains or gates instead of a removable guardrail section at hoist areas if the employer demonstrates the chains or gates provide a level of safety equivalent to guardrails. a.) Loading Dock by large blasting booth - On or about August 25, 2017, Employees using the loading dock to access the fixed ladders to the large blasting booth, were exposed to a fall hazard of over 4 feet due to a missing mid-rail.
