29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Davis-Standard Extruding Line 2:  On, or about, February 2, 2016, the emergency stop provided on the main operating console did not shut down the two Viteck pay-off units.  In the event of an excessive amount of material paying off, employees were required to be in close proximity to the material to shut down the affected pay-off unit using the controls mounted on the side of the pay-off unit.

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized:  In the following locations, on, or about, February 2, 2016, employees conducted Hi-Pot testing on spools of cable using test sets.  The employer had not developed and implemented safe work procedures for high voltage testing stations.  Safe work practices to protect employees from electric shock include, but are not limited to, provided insulated surfaces for the test equipment and for the employees to stand on, providing cut-out switches to disconnect test equipment from the power supply in case of an emergency and isolating the test sets and material being tested to guard against accidental contact.  A.  Test Area Outside of Lab. B.  Kit/Packaging Area.   Abatement Note:  Industry guidelines provide recommendations for the safe design of electrical testing work stations.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  In the following locations, on or about February 2, 2016, employees who performed electrical testing on spools of cable were not provided with personal protective equipment such as, but not limited to, voltage rated gloves and fire resistive clothing:   A.  Test Area Outside of Lab B.  Drum Test Area. C.  Kit/Packaging Area.  Abatement Note for Instances A and C:  The use of a properly designed test area where employees are isolated from electrical hazards may eliminate the need for personal protective equipment.
