Employees operated Yale powered industrial trucks while transporting pallets and loads of twenty foot sections of steel rebar. Employees operated a Wazee Crane 10 ton overhead crane to move bundles of sixty foot lengths of steel rebar, weighing approximately three tons.  Bundle ties were wrapped around the group of rebar to secure the bundle.  The load (bundle of rebar) was attached to the load block hook using the bundle ties.  This condition exposed employees to a crushing hazard.

One employee operated the Hydra Tool Company Inc 175 ton press brake to make stair pans.  The employee placed both hands within 12 inches of the point of operation when forming the stair pan.  The press brake was not provided with machine guarding.  A physical barrier preventing contact with the point of operation was not provided.  The employee had not been provided training with regards to safe work procedures and safe work distance for the press brake.  This condition exposed the employee to an amputation hazard.

Barton Leasing Inc, dba Barton Supply was previously cited for a violation of this Occupational Safety and Health Administration Standard or its equivalent standard, 29 CFR 1910.1200(h)(1), which was contained in OSHA inspection 942481, Citation 1, Item 1c and was issued on January 7, 2014, and affirmed as a final order on January 28, 2014, with respect to a workplace located at 14800 E Moncrieff Place, Aurora, CO.
