29 CFR 1910.23(d)(1)(iii): Stairway(s) less than 44 inches wide having both sides open, did not have one stair railing on each side:  (a) Motor Shop: Employees use a fixed metal staircase 34 inches wide, equipped with 15 risers to gain access to a parts storage loft. The stairway was not equipped with standard railing, in that both the left and right sides ascending did not have intermediate rails and the left side ascending had a section, 9 feet in length that was missing all railing, as observed on February 3, 2016.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) Aultan Shop: An employee operates a Kenna Metal abrasive pedestal grinder (model #104, serial #177401) that was not equipped with an adjustable tongue guard. The abrasive bench grinder had a maximum opening of 1 3/4 inch between the abrasive wheel and the wheel housing, as determined on February 3, 2016.  (b) Machine Shop: An employee operates a Dayton abrasive pedestal grinder (model #471236, serial #XE42X) that had a maximum opening of 1 inch (left side) and 3/8 inch (right side) from the grinding wheel to the tongue guard, as determined on February 3, 2016.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  (a) Motor Shop: An employee used compressed air to clean the work area that was not regulated to 30 p.s.i. or less and had a nozzle that was not equipped with relief ports. The compressed air was measured at 55 p.s.i., as determined on February 3, 2016.   (b) Machine Shop: An employee used compressed air to clean the work area that was not regulated to 30 p.s.i. or less and had a nozzle that was not equipped with relief ports. The compressed air was measured at 99 p.s.i., as determined on February 3, 2016.

29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:  (a) Administrative Office: On September 10, 2015 and times prior to, the employer did not accurately complete column 'F' of the 2015 OSHA Form 300, in that the employer did not include a description of the injury or illness and the object/substance that directly injured or made the person ill. Additionally, the number of days away from work was not accurately maintained per 29 CFR 1904.7(b)(3).
