29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Bridgeport Mill Press: On or about February 25, 2019 and times prior, employees used a Bridgeport mill press to cut key ways. The rotating bit was not provided with a guard to prevent employee exposure to the flying chips and rotating parts. b) Hitachi Mill press: On or about February 25, 2019 and times prior, employees used a Hitachi mill press to cut key ways. The rotating bit was not provided with a guard to prevent employee exposure to the flying chips and rotating parts. c) Clausing Drill Press: On or about February 26, 2019 and times prior, employees used a Clausing Drill Press to drill holes in machine parts. The rotating drill bit was not provided with a guard to prevent employee exposure to the flying chips and rotating parts.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) CNC Area Shop: On or about February 12, 2019 and times prior an employee operated a Kenna Metal abrasive pedestal grinder (model #104, serial #177401) to touch up a bolt. The abrasive bench grinder had a maximum opening of 3/4 inch between the abrasive wheel and the tongue guard, as determined on February 28, 2019. The D&G Machine Company, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.215(b)(9), which was contained in OSHA inspection number 1122698, citation number 1, item number 2 and was affirmed as a final order on February 16, 2016, with respect to a workplace located at 115 Waters St, Mannington, WV.
