29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   (a) Jobsite, American Airlines Hangar 5, Pittsburgh International Airport, Pittsburgh, PA:  On or about February 2, 2016 the employer did not initiate a program that provided for frequent and regular inspections of the worksite by a designated, competent person, to ensure employees were following company programs and procedures and were not exposed to any safety and/or health hazards.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  (a) Jobsite, Pittsburgh International Airport, American Airlines Hangar 5, Pittsburgh, PA:  On or about 02/02/16 an employee did not de-energize a 277v electrical circuit when installing an LED light fixture.  The employee suffered an electrical shock when wiring the fixture sustaining burns and internal injuries to index fingers and thumbs on both hands.  The circuit had not been de-energized prior to the installation and the employee did not use any other insulation methods or personal protective equipment to prevent electrical shock hazards.
