29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Trinity Assembly of God Church, Fairmont WV: On or about May 9, 2018 and times prior, employees were exposed to falls of approximately 18 feet while removing and retrieving tools near the edge of the roof with no fall protection.

29 CFR 1926.1053(a)(24): The side rails of through or sidestep fixed ladders did not extend 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder: (a) Trinity AOG Church, Fairmont WV: On May 9, 2018, and times prior, a sidestep fixed ladder, permanently attached to the building, was used to gain access to and from the roof and did not extend 42 inches above the highest roof level.
