29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles: (a) On or about February 5, 2016, and at times prior, the employer exposed employees to shock hazards in that the power cord on the E-Z Lift conveyor was altered and missing the ground prong. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
