29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301or equivalent) within seven (7) calendar days  of receiving information that a recordable injury or illness had occurred:  A.  On or about February 11, 2016, Pilgrim Furniture City, Southington, Connecticut:  The employer did not record the following workplace injuries and illnesses on the OSHA 300 Log for calendar year 2015:  1.  On or about February 28, 2015, Warehouseman - an employee sliced their finger with a box cutter, resulting in medical treatment beyond first aid, a recordable injury which would have been captured in Column J (Other Recordable Injuries).  2.  On or about March 20, 2015, Store Manager - an employee sustained a lower back strain from dragging a fallen awning, resulting in medical treatment beyond first aid, a recordable injury which would have been captured in Column J (Other Recordable Injuries).  3.  On or about March 27, 2015, Sales - an employee unloading mattresses sustained a torn left shoulder rotator cuff, a recordable injury which would have been captured in Column H (Days Away), resulting in approximately 105 days away from work.  4.  On or about April 9, 2015, Merchandising - an employee slipped on a threshold in the warehouse and sprained their left ankle, a recordable injury which would have been captured in Column I (Restriction/Job Transfer), resulting in approximately 5 days restricted duty.  5.  On or about April 10, 2015, Warehouseman - an employee cut their right index finger with a box cutter.  The injury required stitches, a recordable injury which would have been captured in Column J (Other Recordable Injuries).  6. On or about September 8, 2015, Warehouseman - an employee sustained a back strain injury while lifting a heavy server, a recordable injury which would have been captured in Column I (Restriction/Job Transfer), resulting in approximately 14 days restricted duty.  7. On or about September 24, 2015, Merchandising - an employee sustained a facial laceration while lifting a marble top of a server, a recordable injury which would have been captured in Column H (Days Away), resulting in 3 days away and 3 days restricted duty.  8. On or about November 7, 2015, Warehouseman - an employee sustained a laceration injury to the left index finger from a box cutter, a recordable injury requiring medical treatment beyond first aid, which would have been captured in Column J (Other Recordable Injuries).
