29 CFR 1910.140(c)(20): The employer did not ensure that ropes, belts, lanyards, lifelines, and harnesses used for personal fall protection are protected from being cut, abraded, melted, or otherwise damaged. On or about May 26, 2021, some of the harnesses worn by employees were damaged and had to be replaced due to jammed fasteners or buckles preventing employees from properly securing or loosening the harness, which exposed them to fall injuries as a result of damaged equipment.

On or about May 26, 2021, at approximately 30 feet outside of the paint booth, the employer did not ensure that the containers of flammable liquids were interconnected and bonded while transferring liquids such as the five gallon flammable liquid DuraCoat Pre-Catalyzed Lacquer into its smaller secondary one gallon container, which had a flashpoint of 0�F.
