29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about 02/04/2016, at the jobsite located at La Joya Estates, House # 14675 SW 22nd St, Miami, FL. 33175, two employees were performing roof frame work on a steep roof of 5/12, with unprotected sides and no fall protection, being exposed to fall hazard of approximately 34 feet.     The employer Thales Builders, Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 890698, citation number 1, item 1, and was affirmed as a final order on 07/05/2013, with respect to a workplace located at 10451 NW 58th St. Doral, FL 33178.

29 CFR 1926.25(a):During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  On or about 02/04/2016, at the jobsite located at La Joya Estates, House # 14675 SW 22nd St, Miami, FL. 33175, two employees were conducting work in a construction site with large piles of debris around the building was not kept clear from work areas.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 02/04/2016, at the jobsite located at La Joya Estates, House # 14675 SW 22nd St, Miami, FL. 33175; two employees were accessing the working area of the roof deck using two metal extension ladders that did not extend at least 3 feet above the landing, exposing the employees to a fall hazard of approximately 23 feet and 34 feet.
