29 CFR 1910.132(a): Protective equipment was not used and was not maintained in a sanitary and reliable condition when necessary whenever hazards capable of causing injury and impairment were encountered: a. On or about February 4, 2016, and at times prior thereto, at the Paint Shop, painters did not use personal protective equipment (PPE) that protected the face from skin contact with polyurethane paint aerosols. b. On or about February 4, 2016, and at times prior thereto, at the Paint Shop, cuts were made on painters' Tyvek suits and were not maintained in a reliable condition that protected the skin from contact with polyurethane paint aerosols.

29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: a. On or about February 4, 2016, and at times prior thereto, at the Paint Shop, the employer did not select protective gloves that protected painters' hands from skin contact with polyurethane isocyanates during spray painting. b. On or about February 4, 2016, and at times prior thereto, at the Paint Shop, the employer did not select protective gloves that protected painters' hands from skin contact with chemical components of Type 2 thinner, such as but not limited to methyl ethyl ketone (MEK).
