29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2):    (a)    On or around February 5, 2016, in the Tool Room employees were exposed to amputation hazards when performing servicing of the U-1 Cell Heat Sealer, which involved energizing the machine and manually checking clearance between the heat seal platen stop and the front tray melt stop.  Guards were not in place and no procedure had been established to protect employees from these hazards.
