(a)  On or about October 8, 2019, employees were exposed to a cutting hazard and amputation injury while working on and around Line Four (4) - Cold Saw DCFSW3.  The employer did not ensure interlocks were working to properly guard the point of operation.       In the alternative...    29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a)  On or about October 8, 2019, employees were exposed to a cutting hazard and amputation injury while working on and around Line Four (4) - Cold Saw DCFSW3, in-that specific procedures were not developed and utilized when unjamming the cold saw.      The Tulip Molded Plastic Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1242012, citation number 1, item number 1 and was affirmed as a final order on December 11, 2017, with respect to a workplace located at 714 East Keefe Avenue in Milwaukee, WI 53212.
