29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards: a) On or about January 11, 2016 and time prior; sharp chain chipper head: The employer failed to ensure proper fall protection was used when a maintenance employee climbed onto equipment to oil bearings and connected his lanyard around mid-rail of the catwalk and connected it back to its self which exposed him to fall hazards of 5 feet 6 inches to 8 feet depending on how he would fall.
