29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about February 4, 2016, the employer failed to ensure that effective guarding was in place to protect employees from movement of the hopper dumpers. A simple metal bar was placed around the perimeter of the hazard area. Employees could still reach the hazard area and the bar itself created an additional pinch point.    b) On or about February 4, 2106, the employer failed to ensure that effective guarding was in place for the auger of Hopper 4 in the Wolf King area of the facility. Employees were observed reaching over the railing above the auger to remove food products from the vat.
