29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter. On or about February 10, 2016, the employer failed to protect employees from fire and smoke exposure hazards when it did not provide training for employees on the proper use of portable fire extinguishers.

On or about February 10, 2016, the employer failed to protect employees from struck by and crushing hazards due to the damage of the outside columns and cross support sections including, but not limited to, aisle 42 in the center of the warehouse.
