And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017;  On or about August 16, 2021 the employer failed to ensure that employees tasked with the use of employer supplied PFAS and other fall protection devices were trained on such prior to their use. The employer provides powered industrial elevated order pickers as well as a manufactured steel man baskets (fork mounted) for use in reaching racking systems. The employer requires the use of PFAS when these are utilized however has not trained employees in fall protection.
