29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): (a) On or about February 9, 2016, at an excavation located at 230 Airport Rd. Shawano, WI, employees working in an approximately 8 ft. deep, 24 ft. long, and 15 ft. wide excavation were exposed to cave-in hazards because cave-in protection was not being used.
