29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c):    DeGroot Construction project site located at the intersection of Oak St and Madison Ave, Sturgeon Bay, WI: On April 7, 2016, two employees were exposed to struck by hazards while working in an excavation without cave-in protection where the end wall was not properly sloped.
