29 CFR 1910.27(c)(4): "Clearance in back of ladder." The distance from the centerline of rungs, cleats, or steps to the nearest permanent object in back of the ladder were less than 7 inches. a) Date of Violation: On or about 01-29-16 and times prior thereto, Location: Building#V6A Portside at 4000 Coast Guard Blvd, U.S. Coast Guard Base Portsmouth, VA 23703: Employee(s) were exposed to fall hazards from 18 ft., 7 inches by using an improperly installed fixed interior ladder. The fixed ladder was installed whereas the distance from the center of the rung(s) to the permanent object (wall) in the back was only six inches. A contract employee had fallen and resulted in an in-patient hospitalization. There was a total of four (4) improperly installed ladders across the facility in various buildings. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
