Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1960.8(a): 29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:    a)    United Coast Guard: Remote Antenna Site, Virginia Beach: On or about 07/05/2016, an employee was exposed to induced current as result of non-ionizing from antenna NX01. The antenna was not maintained according to the Institute of Electrical and Electronics Engineers Standard C95.1 2005, section 4.2.3 for Safety Levels with Respect to Human Exposure to Radio Frequency Electromagnetic Fields, 3 kHz to 300 GHz. The barrier used to protect employees from exposure was destroyed in 2012 during a storm.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or likely to be present, which necessitate the use of personal protective equipment.      a)    United States Coast Guard: On or about 07/05/2016, a hazard assessment at the remote antenna site in Virginia Beach was not conducted as a result an employee was exposed to possible injuries.      Notice #1, Item #1, 29 CFR 1910.132(d)(1) required by 29 CFR 1960.8(b): The employer did not conduct a hazard assessment for employees exposed to hot tar dropping from the roof. The employer was cited for a violation of the same standard on 06/19/2014, Inspection number 978665, 07/16/014 of final order/settlement, and means of abatement.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
